Morton is a patient-centered medical home (PCMH). It is a model of care that puts the patient at the forefront of care. PCMH builds better relationships between the patient and their clinical care teams.
The patient-centered medical home model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach. The PCMH allows Morton to coordinate patient care across the health system.
The Health Center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Morton Comprehensive Health Services receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
